TaMyra King-Carter started as a LTSS-Waiver Care Coordinator for Complete Care Management Partners, LLC in December 2020 and LTSS-Waiver Manager in January 2022. From there, TaMyra transitioned into the role of Transitional Care Management (TCM) Manager in January 2023. She brings more than 8 years of care management experience to Complete Care.
As a TCM manager, TaMyra is responsible for leading the TCM team whose primary role is to identify and address client-members’ needs as they transition from acute care settings back to home or other community-based places. TaMyra guides the TCM team as they assist client-members through engagement, assessment, care planning, and care coordination, including ensuring any follow-up appointment, medication, home health, and durable medical equipment needs are met. She focuses the team on improving transitions across settings, thereby minimizing post-discharge risks and preventable readmissions. TaMyra and the TCM team are also responsible for assisting with locating hard to reach high utilizing client-members at emergency room and inpatient settings.
Prior to Complete Care, TaMyra spent 3 years serving in various roles at NextLevel Health Partners, including care manager, care coordinator, and LTSS-waiver care manager. TaMyra has been an RN for over ten years and with an extensive background in transitional care management, care management with a Managed Care Organization (MCO), Long Term Rehabilitation services, hospice care, and other varied roles at MCO’s, hospitals, long term care facilities, and home health agencies.
TaMyra is a determined leader that takes pride in the services and assistance that is being provided. She is also extremely passionate about making a difference whenever possible, one day at a time, one person at a time. She has a bachelor’s degree of Science in Nursing. Leading the TCM team for Complete Care Management Partners provides the perfect opportunity for Mrs. King-Carter to demonstrate her passion for advocacy, health education, and improvement in health care for communities in need.
TaMyra is a daughter, sister, wife, and mother of three beautiful girls. She mostly enjoys listening to live music, cooking, trying out new restaurants, spending quality time with her family and friends, and traveling. She has recently found a passion for Autism advocacy and enjoys learning about the Special Needs Community.
